About the role

Key responsibilities & impact
  • Ensure patient care is coordinated and managed appropriately
  • Serve as the first point of contact for field clinicians and provide timely support
  • Ensure care and services are delivered appropriately
  • Supervise clinical personnel and oversee clinicians providing in-home patient care
  • Provide in-the-field oversight, including performance evaluations and hiring
  • Handle incoming calls and follow up to meet internal and external customer needs
  • Coordinate training and educational programs to improve clinical outcomes and competencies
  • Assess staff learning needs and formulate plans and goals for evaluation periods
  • Perform administrative call duties and serve as a role model and resource for staff
  • Assist clinicians with patient care escalations
  • Coordinate at least bi-weekly case conferences to discuss patient progress and revise care plans
  • Review and evaluate cases, clinical services, records, diagnoses, medications, procedures, and clinical courses
  • Guide clinicians toward effective performance and quality home care services
  • Assist clinicians in establishing therapeutic goals, setting priorities, and developing plans of care

Requirements

What you’ll need
  • Two years management or supervisory experience in a rehabilitation therapy department or program
  • Home Health Experience Highly Preferred
  • Possesses a degree from a baccalaureate or masters program in applicable discipline approved by an accredited organization
  • Current CA Occupational Therapist license
  • Current Basic Life Support (BLS)
  • Valid CA Drivers License with acceptable driving record
  • All license and certifications must be current at time of hire and sustained throughout employment
  • Ability to work 2 days in the Roseville or Natomas office and 3 days in the field
  • Ability to visit an average of 15 patients each week
