About the role
Key responsibilities & impact- Ensure patient care is coordinated and managed appropriately
- Serve as the first point of contact for field clinicians and support patient care escalations
- Ensure care and services are delivered appropriately
- Supervise clinical personnel and oversee clinicians providing in-home patient care
- Conduct field oversight of clinicians, including performance evaluations and hiring
- Handle incoming calls and follow up to meet internal and external customer needs
- Coordinate training and educational programs for field nurses
- Assess staff learning needs and formulate plans and goals for evaluation periods
- Perform administrative call duties and serve as a role model and resource for staff
- Coordinate at least bi-weekly case conferences to discuss patient progress and revise care plans
- Review and evaluate cases, clinical services, diagnoses, medications, procedures, and clinical courses
- Guide clinicians to improve performance and quality of home care services
- Assist clinicians with therapeutic goals, priorities, and plans of care
Requirements
What you’ll need- Two years management or supervisory experience in a rehabilitation therapy department or program
- Home Health Experience Highly Preferred
- Possesses a degree from a baccalaureate or masters program in applicable discipline approved by an accredited organization
- Current CA Occupational Therapist license
- Current Basic Life Support (BLS)
- Valid CA Drivers License with acceptable driving record
- All licenses and certifications must be current at time of hire and sustained throughout employment
- Ability to work 2 days in the Roseville or Natomas office and 3 days in the field
- Ability to visit an average of 15 patients each week
