About the role
Key responsibilities & impact- Provide field oversight of clinicians caring for patients in their homes
- Coordinate and manage patient care and services
- Serve as the first point of contact for field clinicians and assist with patient-care escalations
- Handle incoming calls and follow up to meet customer needs
- Coordinate training and educational programs for field nurses
- Assess staff learning needs and formulate evaluation-period goals
- Perform administrative call duties and serve as a role model and resource
- Coordinate at least bi-weekly case conferences to discuss patient progress and revise care plans
- Review and evaluate cases, services provided, clinical records, diagnoses, medications, procedures, and clinical courses
- Guide clinicians toward effective performance and quality home-care delivery
- Assist clinicians with therapeutic goals, priorities, and plans of care
- Work two days in the Roseville or Natomas office and three days in the field, averaging 15+ patient visits weekly
Requirements
What you’ll need- Two years of management or supervisory experience in a rehabilitation therapy department or program, preferably home care
- Degree from a baccalaureate or master's program in an applicable discipline approved by an accredited organization
- Current California Physical Therapist license
- Current Basic Life Support (BLS) certification
- Valid California driver's license with acceptable driving record
- All licenses and certifications must be current at hire and maintained throughout employment
