About the role

Key responsibilities & impact

Assist, under the supervision of a Physical Therapist, in providing quality patient care in patients' homes
Address patient needs so individuals can live safely and productively in their home environment
Examine each individual and develop treatment plans using techniques to promote mobility, reduce pain, restore function, and prevent disability
Develop fitness- and wellness-oriented programs to prevent loss of mobility and promote healthier, more active lifestyles
Visit patients in their homes throughout the anticipated San Rafael/Marin County territory

Requirements

What you’ll need

License PTA in California
Home Health experience is highly preferred
Current CPR License
Current CA Driver's License
Computer Proficiency
Strong organization and communication skills
Work well independently and in a team
Excel at building rapport with patients and providing the highest quality of care
Experience with high acuity patients is a plus
Experience working with an interdisciplinary team is highly desired
Case Management fundamentals is highly desired
Self-Directed
Empathetic
Critical thinker
Outstanding interpersonal skills

Required to complete a computer literacy assessment before scheduling an in-person interview
