About the role

Key responsibilities & impact
  • Assess patients and develop plans of care according to patient needs, physician orders, and agency policies
  • Assist with ongoing patient assessments and carry out plans of care
  • Maintain the home chart during each assignment according to clinical and nursing procedures
  • Initiate appropriate preventive and rehabilitative nursing procedures
  • Counsel and instruct patients and families
  • Complete and accurately submit timesheets and nursing clinical notes
  • Liaise between the agency, patients, and physicians
  • Communicate with patients, supervisors, and physicians
  • Report changes and findings to physicians and supervisors promptly
  • Provide patient- and family-centered home health care as part of a multidisciplinary team

Requirements

What you’ll need
  • LVN qualification
  • Minimum of one year experience in a home health agency or community health setting
  • Clinical knowledge and critical thinking ability to plan and coordinate patient care
  • Exceptional interpersonal and customer service skills
  • Ability to solve unique problems or identify when to consult the intake supervisor
  • Knowledge of current Medicare, Medi-Cal, State and Federal regulations
  • Ability to communicate clearly with patients, supervisors, and physicians
  • Ability to work in the Santa Clara County service area, including Milpitas, Sunnyvale, Mountain View, Santa Clara, and San Jose
