Requirements

  • Ability to draft care plans and reconcile patient medications
  • Ability to ensure orders for care are accurate and complete
  • Ability to prepare accurate, orderly, and timely documentation
  • Ability to coordinate care with patients and provide reports to the Case Manager
  • Ability to provide health care instructions to patients and families according to assessment and plan
