About the role
Key responsibilities & impact- Provide direct patient care in patients’ homes in Green Tree, Pittsburgh, and surrounding areas
- Conduct Start of Care comprehensive assessments and complete OASIS timely
- Draft care plans and reconcile patient medications
- Ensure orders for care are accurate and complete
- Prepare accurate, orderly, and timely documentation
- Coordinate care with patients and provide reports to the Case Manager
- Provide health care instructions to patients and families according to assessment and plan
Requirements
What you’ll need- Active RN license in the state in which practicing
- New graduates with eligibility to take the NCLEX may qualify
- Ability to conduct comprehensive Start of Care assessments and complete OASIS timely
- Ability to draft care plans and reconcile patient medications
- Ability to prepare accurate, orderly, and timely documentation
- Ability to coordinate care with patients and provide reports to the Case Manager
- Ability to provide health care instructions to patients and families according to assessment and plan
