About the role
Key responsibilities & impact- Provide direct patient care in patients' homes in the Tamaqua/Jim Thorpe, Pennsylvania area and surrounding areas
- Conduct Start of Care comprehensive assessments and complete OASIS documentation timely
- Draft care plans and reconcile patient medications
- Ensure care orders are accurate and complete
- Prepare accurate, orderly, and timely documentation
- Coordinate care with patients and provide proper reports to the Case Manager
- Provide health care instructions to patients and families according to the assessment and plan
Requirements
What you’ll need- Active RN license in the state in which practicing, or new graduate with eligibility to take the NCLEX
- Ability to conduct comprehensive Start of Care assessments and complete OASIS timely
- Ability to draft care plans and reconcile patient medications
- Ability to prepare accurate, orderly, and timely documentation
- Ability to coordinate care and provide reports to the Case Manager
- Ability to provide health care instructions to patients and families
