About the role
Key responsibilities & impact- Provide skilled nursing care to hospice patients in their homes or in a hospice facility
- Assess patient needs and develop individualized care plans in collaboration with the interdisciplinary team
- Administer medication, monitor vital signs, and manage symptoms to ensure patient comfort
- Educate and support patients and their families regarding end-of-life care and symptom management
- Coordinate care with other healthcare professionals and community resources
- Offer emotional support, counseling, and education to patients and their families, addressing their unique needs and concerns
Requirements
What you’ll need- Active RN license in the state in which you are practicing, or be a new graduate with eligibility to take NCLEX
- Minimum 1 year of experience in hospice, palliative care, or related field preferred but not required
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal abilities
- Ability to work collaboratively in a multidisciplinary team
- Compassionate and empathetic approach to patient care
- Knowledge of hospice regulations and practices preferred
