About the role

Key responsibilities & impact
  • Conduct start-of-care comprehensive assessments
  • Draft care plans and reconcile patient medications
  • Ensure orders for care are accurate and complete

Requirements

What you’ll need
  • Active RN license in the state in which practicing
  • Minimum of 1 year of experience preferred
  • Strong clinical assessment and critical thinking skills
  • Excellent communication and interpersonal skills
  • Compassionate and empathetic approach to patient care
  • Ability to work independently and as part of a team
  • Knowledge of hospice regulations and standards of care
  • Reliable transportation and willingness to travel
