About the role

Key responsibilities & impact
  • Perform comprehensive Physical Therapy evaluations
  • Develop individualized plans of care and functional goals
  • Address strength, balance, gait, transfers, endurance and functional mobility
  • Provide therapeutic exercise and functional mobility training
  • Develop fall-prevention and safety strategies
  • Assess the need for walkers and other assistive devices
  • Monitor patient progress and modify plans of care when appropriate
  • Supervise and collaborate with Physical Therapist Assistants
  • Educate patients, families and caregivers
  • Collaborate with the interdisciplinary care team
  • Complete accurate and timely clinical documentation
  • Evaluate and treat patients with mobility and functional needs
  • Follow patients' progress from evaluation through treatment and recovery

Requirements

What you’ll need
  • Graduate of an accredited Physical Therapy program
  • Current or eligible Pennsylvania Physical Therapist license
  • Strong clinical judgment and communication skills
  • Ability to collaborate effectively with PTAs and the interdisciplinary team
  • A patient-first approach to therapy
  • Skilled nursing, geriatric or rehabilitation experience is a plus, but not required
  • Physical Therapy clinical knowledge
  • Ability to work with adult and geriatric populations
  • Ability to work with medically complex patients
