About the role

Key responsibilities & impact
  • Perform comprehensive Physical Therapy evaluations
  • Develop individualized plans of care based on patient needs and goals
  • Address strength, balance, gait, transfers, endurance and functional mobility
  • Provide therapeutic exercise, neuromuscular re-education and functional mobility training
  • Assess the need for assistive devices and provide appropriate training
  • Develop fall-prevention and safety strategies
  • Monitor patient progress and modify treatment plans when appropriate
  • Educate patients, families and caregivers
  • Collaborate with PTAs and the interdisciplinary care team
  • Complete accurate and timely clinical documentation
  • Help patients work toward greater safety, mobility and independence
  • Care for adult and geriatric patients in a skilled nursing and rehabilitation setting

Requirements

What you’ll need
  • Graduate of an accredited Physical Therapy program
  • Current or eligible Ohio Physical Therapist license
  • Strong clinical judgment and communication skills
  • Ability to work collaboratively with PTAs and the interdisciplinary team
  • Skilled nursing, geriatric or rehabilitation experience is a plus
  • A patient-first approach to care
