About the role

Key responsibilities & impact
  • Coordinate direct and indirect patient care based on the nursing process
  • Perform telephone triage and patient assessment from the Agrace office or home
  • Assess calls and determine whether a home visit or telephone assistance is needed
  • Respond to patient and family calls in a timely manner
  • Triage patient and family needs by phone and assess the need for home visits
  • Communicate with patients, families, collaborating agencies, healthcare providers, and interdisciplinary team members
  • Use patient, family, and clinical information for assessment and care coordination
  • Apply knowledge of uncomplicated grief and the dying process in assessment, care planning, and intervention
  • Identify and report abuse, neglect, suicidal ideation, or violence
  • Provide individualized patient and family education
  • Develop and document acute care plans
  • Assess pain continuously and collaborate on pain management during crises
  • Participate in mandatory education
  • Precept new staff

Requirements

What you’ll need
  • ADN required; BSN preferred
  • Current, unrestricted Registered Nursing License in state of employment
  • One-year nursing experience preferred
  • Must be at least 18 years of age
  • Ability to develop, maintain, and apply knowledge of hospice and end of life process
  • Ability to maintain professional boundaries with patients, families and co-workers
  • Ability to create and follow plans of care, and patient, family, and clinical staff directions
  • Basic Microsoft Office skills
  • Intermediate electronic health records skills; Epic preferred
  • Ability to work effectively and cooperatively with others
  • Ability to communicate effectively verbally and in writing
  • Ability to be flexible in time and task to meet organizational needs
  • Ability to maintain confidentiality about patients, families, caregivers, staff, donors, and organizational intellectual property
  • Ability to use proper body mechanics, safe patient handling, and standard precautions
  • Must live within 60 miles of the Madison, WI office
