About the role
Key responsibilities & impact- Conduct in-home assessments for Alignment members two days per week, including medication review and health screenings
- Work from home three days per week
- Provide patient education by matching desired care with best care
- Identify diagnoses for care management and active medical management
- Partner with the Regional Medical Officer and other team members to develop appropriate care plans
- Coordinate care with PCPs, specialists, ancillary providers, and other stakeholders
- Lead a broader clinical team that may include nurses, health coaches, social workers, and care coordinators
- Provide care and support to eligible Alignment Healthcare patients in their homes to help prevent unnecessary hospitalizations, health complications, and unmanaged disease progression
Requirements
What you’ll need- Required: Master’s degree from an accredited NP Program or PA program
- Required: Active Nurse Practitioner or Physician Assistant license for the state of California
- Required: Active Nurse Practitioner Board Certification/Physician Assistant Certification
- Required: NPI Number, DEA, Valid BLS
- Required: Valid Driver License and current automobile insurance
- Required: Ability to communicate positively, professionally and effectively with others
- Required: Effective problem-solving, organizational and time-management skills and ability to work in a fast-paced environment
- Preferred: One (1) year of prior clinical or home care experience
- Preferred: Previous EMR experience
- Preferred: Experience in care of older adult (geriatric) patients
- Ability to meet the stated physical demands, including lifting and/or moving up to 20 pounds
