About the role

Key responsibilities & impact
  • Assure integrated whole-person care management for individuals and families with special health care needs
  • Coordinate physical health, behavioral health, pharmacy, and unmet health-related resource needs
  • Complete comprehensive assessments and Care Needs Screenings
  • Develop and update Plans of Care
  • Assign interventions and care activities to appropriate care management team members and Community Health Workers
  • Submit referrals for medically or pharmaceutically complex needs
  • Assist members or legally responsible persons in choosing service providers
  • Evaluate service appropriateness and monitor implementation of Plans of Care
  • Use person-centered planning, motivational interviewing, and JIVA assessment history
  • Collaborate with members, guardians, care teams, service providers, community systems, and stakeholders
  • Schedule and conduct face-to-face, virtual, and telephone meetings
  • Educate members about Alliance Health Plan, rights, grievances and appeals, services, providers, and payer requirements
  • Recognize and report critical incidents and provider quality concerns
  • Coordinate transitions to appropriate levels of care and community-based services
  • Identify treatment barriers and assist with appointments and treatment-provider linkages
  • Maintain required member contacts and attend community, provider, and stakeholder meetings
  • Obtain, upload, and maintain supporting documentation and releases of information
  • Open episodes and maintain member records in JIVA, Clive, and TCLD
  • Escalate complex cases and ensure clinical documentation meets state, agency, and Medicaid requirements
  • Complete TCL monitoring requirements supporting tenancy, health, safety, community integration, housing, and provider coordination
  • Support supportive-housing tenancy stability, voucher applications, rehousing, bridge/hotel-program transitions, and housing separations
  • Educate members about IPS-SE employment, community integration, and education referrals
  • Travel between Alliance offices and to meetings, member/provider/stakeholder appointments, court hearings, and Alliance-sponsored events

Requirements

What you’ll need
  • Bachelor’s degree from an accredited college or university in Human Services field and two (2) years of post-bachelor’s degree mh/dd/sa experience with the population served, OR Bachelor’s degree in a Non-Human Services field and four (4) years of post-bachelor’s degree mh/dd/sa experience with the population served, OR Master’s Degree in Human Services and one (1) year of post graduate degree mh/dd/sa experience with the population served
  • Fully or Provisionally Licensed in the State of North Carolina as a LCSW, LCMHC, LPA, or LMFT, OR Licensed Registered Nurse (RN) in the State of North Carolina with four (4) years of mh/dd/sa experience with the population served
  • Preferred: NACCM, NADD-Specialist and/or CBIS Certification
  • Knowledge of Person Centered Thinking/planning
  • Knowledge of using assessments to develop plans of care
  • Knowledge of Diagnostic and Statistical Manual of Mental Disorders
  • Knowledge of LOC process, SIS for IDD and FASN assessment for TBI
  • Knowledge of Medicaid Tailored Plan, Medicaid Direct, enhanced MHSUD, and waiver benefits plans
  • Knowledge of and skilled in the use of Motivational Interviewing
  • Proficient in Microsoft Office products (such as Word, Excel, Outlook, etc.)
  • Strong interpersonal and written/verbal communication skills, including conflict management and resolution skills
  • High level of diplomacy and discretion
  • Ability to make prompt, independent decisions based upon relevant facts
  • Employment contingent upon satisfactory background and MVR (Motor Vehicle Registration) check
  • Ability to travel between Alliance offices and to meet with members, providers, stakeholders, and attend court hearings as required
