About the role

Key responsibilities & impact
  • Provide care team support for members transitioning from inpatient settings to appropriate lower or lateral levels of care
  • Provide subject matter expertise within scope regarding members' physical and/or behavioral health to support whole-person care management
  • Collaborate with Alliance staff, behavioral health providers, primary care physicians, specialty care providers, community partners and stakeholders
  • Support High Priority Reentry Liaison Transition Coordinator functions
  • Meet with clients regularly, including travel, to provide one-on-one support and counseling
  • Develop recovery plans, short-term goals, coping methods and rehabilitation strategies with clients
  • Arrange medical appointments, therapy sessions, support groups and other resources
  • Report unusual behavior, conflicts, recovery deviations, challenges, conflicts or barriers to relevant professionals and the High Priority Reentry Liaison Transition Coordinator
  • Liaise regularly with family members and medical professionals
  • Coordinate client logistics and address barriers to appointments, treatments and services
  • Travel between Alliance offices, attend meetings and Alliance-sponsored events, meet members and providers, engage stakeholders, and attend court hearings as required

Requirements

What you’ll need
  • High School Diploma with two (2) years of experience working with people with SMI/SPMI and their families or guardians
  • Lived experience with mental health conditions, substance abuse or trauma is required
  • NC Certified Peer Support Specialist Program certification is required
  • Computer proficiency in Microsoft Excel, Word and Outlook
  • Ability to utilize computer equipment and web-based software to conduct work
  • Ability to interact with various office staff as needed to support necessary workflows
  • Ability to interact with healthcare professionals, patients, their families and other supports
  • Ability to communicate effectively to individuals and groups through spoken, written and electronic media
  • Ability to attend to detail, effectively prioritize and execute tasks in a timely manner
  • Ability to work independently without a high degree of supervision
  • Employment contingent upon a satisfactory background and MVR (Motor Vehicle Registration) check
  • Must reside in North Carolina within 40 miles of one of the counties within the Alliance catchment areas (Mecklenburg, Wake, Durham, Orange, Johnston, Harnett, or Cumberland)
  • Experience providing peer supports to individuals in and out of the Justice System preferred
