About the role

Key responsibilities & impact
  • Conduct nutritional assessments of patients with diabetes
  • Develop nutrition plans
  • Educate patients, families, staff and other caregivers about nutrition in diabetes management
  • Collaborate with physicians to identify nutritional and/or diabetes management needs and plan care
  • Utilize approved protocols for medication management when appropriate
  • Provide consultation and mentoring related to diabetes care to staff in complex cases and unfamiliar clinical problems
  • Partner and collaborate with the multidisciplinary team
  • Plan and document discharge needs for patients and families
  • Conduct patient and family education and evaluate responses
  • Document assessments, plans, interventions, education, outcomes, nutritional goals and data in the medical record
  • Document to ensure accurate coding and billing for services
  • Maintain documentation tools, educational literature and resources
  • Perform other duties as assigned

Requirements

What you’ll need
  • Bachelor's degree in Dietetics or related field
  • 2 to 5 years of experience in treatment of diabetes patients in hospital or nursing home setting
  • 2 to 5 years in documentation in electronic medical record
  • Licensed Dietitian - Minnesota Board of Dietetics and Nutrition Practice required
  • Licensed in Wisconsin for WI practice settings upon hire
  • Registered Dietitian - Commission on Dietetic Registration
  • Academy of Nutrition and Dietetics preferred
  • American Dietetic Association Registration upon hire
  • Certified Diabetes Educator - National Certification Board for Diabetes Educators required within 4 years
  • American Dietetic Association Registration upon hire preferred
  • Licensed Dietician-Wisconsin for Dieticians in Wisconsin practice setting upon hire required
  • Ability to perform sedentary work, lifting up to 10 lbs. occasionally and negligible weight frequently
