About the role
Key responsibilities & impact- Provides clinical care for patients
- Completes comprehensive patient assessments
- Determines diagnosis and treatment of illness
- Orders and interprets medical diagnostics in tandem with attending physician
- Prescribes medications, treatments, therapy and medical equipment
- Conducts patient education for chronic illness or wellness care
- Refers patients for specialty care when determined
- Consults with physicians when needs exceed scope of practice or diagnosis/treatment plan is unclear
- Promotes communication within the health care team and interdisciplinary staff
- Acts as a resource to team members
- Works with primary care team for program development
- Acts as liaison with outside agencies as needed
- Collaborates with hospital staff by consulting and rounding on patients when designated by the specialty
- Works in inpatient and outpatient settings independently or as part of a treatment team
- Serves as educator, case manager and consultant to staff and multidisciplinary health care team
- Performs other duties as assigned
Requirements
What you’ll need- Master's degree - Completion of accredited Nurse Practitioner program
- Licensed Registered Nurse - MN Board of Nursing required upon hire
- Certified Nurse Practitioner - American Nurse Credentialing Center (ANCC) certification within specialty required upon hire
- Certified Nurse Practitioner - MN Board of Nursing required upon hire
- ACLS-BLS Tier 2 - Multisource required within 180 Days
- Drug Enforcement Administration required upon hire
- Valid Driver’s License required upon hire
- New Graduates may be considered
- Minimum 2 years experience in palliative care, internal medicine, family practice, hospital medicine, or oncology
- MN and WI residents must obtain a valid driver’s license in their state of residence within 60 days of employment for those working in Complex Care for Seniors, Community Palliative Care and Hospice
- Ability to lift up to 32 lbs. occasionally and up to 25 lbs. frequently
