About the role
Key responsibilities & impact- Provide day-to-day clinical leadership, guidance, and support to PTs, OTs, and SLPs
- Serve as a clinical resource for patient needs, changes in condition, care planning, documentation, and clinical questions
- Support clinician development through coaching, education, feedback, onboarding, orientation, and ongoing communication
- Assist with patient visits and clinical coverage when needed
- Oversee and coordinate therapy services from admission through discharge
- Ensure timely delivery of therapy services based on patient needs and plans of care
- Address changes in patient status, scheduling challenges, missed visits, and barriers to care
- Coordinate communication among physicians, care managers, clinicians, patients, families, and internal partners
- Support appropriate utilization of therapy services and patient outcomes
- Oversee clinical documentation, including OASIS requirements as applicable
- Ensure documentation is timely, accurate, complete, and compliant with regulatory, payer, and organizational requirements
- Identify documentation and clinical practice trends and provide coaching or education
- Reinforce clinical policies, procedures, standards, and home health regulations
- Promote readiness for audits, surveys, and regulatory requirements
- Partner with clinical, scheduling, care management, recruiting, and operational teams on patient coverage and clinician utilization
- Identify gaps and drive solutions for coverage, workflow, communication, and care delivery
- Escalate barriers and balance patient needs, clinician capacity, operational priorities, and regulatory requirements
- Participate in team meetings, cross-functional initiatives, and process improvements
Requirements
What you’ll need- Current, unrestricted Colorado license as a Physical Therapist, Occupational Therapist, or Speech-Language Pathologist
- Professional experience providing therapy services to adult and/or geriatric populations
- Home health experience with a strong understanding of adult therapy-based care delivered in the home
- Experience with OASIS documentation and home health clinical documentation requirements
- Strong clinical judgment and ability to independently evaluate and navigate patient-care concerns
- Excellent communication, organization, and time-management skills
- Ability to prioritize competing clinical and operational needs in a fast-paced environment
- Strong interpersonal skills with clinicians, patients, families, physicians, and cross-functional partners
- Ability to work independently while remaining collaborative and team-oriented
- Previous supervisory, lead, preceptor, or clinical leadership experience preferred
- Experience coaching, mentoring, or educating therapy clinicians preferred
- Experience overseeing a multidisciplinary therapy team preferred
- Working knowledge of Medicare-certified home health operations, documentation, and regulatory expectations preferred
- Experience supporting quality improvement, clinical compliance, or operational initiatives within home health preferred
