About the role
Key responsibilities & impact- Assess patients' social, emotional, and environmental needs to build personalized intervention plans
- Coordinate post-discharge transitions through concurrent reviews, discharge planning, home care, and continuing social services
- Connect patients and families with resources by collaborating with community agencies, healthcare partners, and support networks
- Consult with clinical managers, external organizations, and loved ones to align care strategies and ensure treatment compliance
Requirements
What you’ll need- Social Worker credentialed from the Maryland Board of Social Work Examiners obtained prior to hire date or job transfer date; any level accepted
- Bachelor's Degree in Social Work required at date of hire
- Monday-Friday 8:00AM-4:30PM schedule
- No weekends or holidays
- Employer does not participate in E-Verify and therefore cannot employ STEM OPT candidates
