About the role

Key responsibilities & impact
  • Conduct outreach and assessments; enroll patients and provide ongoing assessment and case management services
  • Advocate for patients by identifying unmet needs and removing barriers to health and wellness
  • Communicate with patients, document in medical records, and interface with medical providers
  • Work in medical offices, specialists’ offices, hospitals, and patients’ homes with minimal supervision
  • Assist in developing patient-centered care plans
  • Connect patients with internal and community resources
  • Serve as a liaison between community health and social service providers and Asian Health Services
  • Collaborate with clinic staff, community members, and the multidisciplinary Care Neighborhood team
  • Report suspected abuse and neglect as a mandated reporter
  • Attend required AHS and CHCN meetings, training, and the Care Neighborhood patient support group RESPECT
  • Meet departmental and Care Neighborhood documentation standards
  • Build and maintain relationships on behalf of CHCN
  • Participate in agency outreach, advocacy, events, quality improvement processes, and performance improvement teams
  • Perform other duties as assigned

Requirements

What you’ll need
  • Bachelor level in Social Work or related field, or at least three years of work experience in a community-based health care capacity
  • Fluent in Cantonese and/or Vietnamese is required
  • Competent leadership and administrative skills
  • Flexibility and creativity
  • Good communication and customer relations skills; ability to work well with a team and independently
  • Proficiency in computer operations, navigational skills, and comfort with Internet-based applications
  • Sound decision-making skills including problem-solving, critical thinking, and good judgment
  • Professional demeanor
  • Excellent communication, organizational, and time management skills with the ability to meet tight timeframes
  • Strong understanding of the managed care environment
  • Ability to work effectively and collaboratively with hospital/office staff, health plans, members, and physicians
  • Flexible and adaptable to change
  • Ability to learn quickly and retain complex information
  • Travel to work assignments
  • Valid current license and appropriate individual automobile insurance must be maintained
