About the role

Key responsibilities & impact
  • Comply with Astrana and department policies and procedures
  • Assess members' medical, social, and behavioral needs
  • Complete comprehensive member assessments within required timeframes and identify candidates for case management
  • Document accurately and completely in all systems
  • Develop and implement individualized case management plans with members, caregivers, social workers, physicians, healthcare professionals, and support persons
  • Collaborate with hospitalists, inpatient case managers, and discharge planners to establish safe discharge plans beginning at admission
  • Review medical records and documentation to determine medical necessity
  • Conduct post-discharge medication reconciliation
  • Review medications for accuracy, omissions, duplications, and potential interactions
  • Promote medication adherence through reminders, education, and barrier reduction
  • Close open quality measure and care gaps
  • Attend company, team, and interdisciplinary team meetings
  • Perform other assigned duties

Requirements

What you’ll need
  • Active Licensed Vocational Nurse (LVN) license in California
  • Ability to become licensed in other states, including NV, AZ, and HI
  • Minimum 1 year of clinical experience in acute care, ambulatory care, case management, or related experience
  • Experience in a managed care environment, including a health plan, IPA, or MSO
  • Proficiency with Microsoft Office applications
  • Experience navigating electronic medical record (EMR), care management, or other healthcare technology platforms
  • Bilingual in Mandarin/Cantonese or Spanish is preferred
  • At least 1 year of Case Manager experience is preferred
  • Basic knowledge of ICE, DHS, DMHC, NCQA, and CMS standards is preferred
  • Previous case management experience including BCM, CCM, SNP, ECM, and TOC/TCS is preferred
  • Ability to work across multiple technology platforms and learn and adapt to new tools and workflows
