About the role

Key responsibilities & impact
  • Provide home visits to at-risk patients and families to evaluate needs and direct care toward desired outcomes cost-effectively
  • Report specific health and social information to the care management team to assist in developing care pathways
  • Participate in multidisciplinary team conferences as needed
  • Assist patients with accessing needed services
  • Recognize potentially serious problems to prevent poor health and social outcomes
  • Provide a link between patients and available resources to deliver necessary resources cost-effectively

Requirements

What you’ll need
  • High school diploma or equivalent
  • Minimum of 1 year of experience working with a community agency providing health care services, including community resources, communication skills, individual and family advocacy, health education, and service skills
  • Active patient of the state's Community Health Worker Association
  • Effective communication skills
  • Extensive experience with Microsoft Office Suite (Word, PowerPoint, Excel)
  • Professional work ethic
  • Strong comfort with working in a fast-paced environment
  • Authentic passion for improving the lives of underserved patients and advancing value-based care
  • Community Health Worker (CHW) certification through a college-level program
  • Ohio Plan: CHW certification through a college-level program or LPN or LCSW license required
  • Michigan Plan: CHW certification preferred but not required
  • Illinois Plan: CHW certification preferred but not required
  • Medicare: CHW certification through a college-level program or LPN license required
  • Valid driver's license required
  • Kentucky Plan: CHW certification preferred but not required
