About the role

Key responsibilities & impact
  • Provide strategic and accountable leadership for care and services across assigned facilities
  • Establish, implement, and ensure adherence to policies and procedures
  • Coordinate orientation programs for new staff
  • Develop and engage existing staff
  • Lead quality improvement activities, develop monitoring tools, collect data, and use data to improve care and services
  • Oversee total nursing care of residents and accuracy of nursing care records
  • Participate in prospective resident selection based on nursing needs and available personnel competencies
  • Oversee Federal and State SNF Conditions of Participation, including admissions, MDS/care planning, restorative nursing, and care delivery
  • Establish policies and procedures for resident care, including nursing activities, social services, and delegated medical care
  • Determine nursing personnel numbers and levels with the Administrator
  • Establish staffing patterns for 24-hour quality nursing care
  • Participate in facility planning, decision-making, and budgeting
  • Develop safety and quality-of-care policies
  • Coordinate the QAPI program and facilitate staff participation
  • Delegate and monitor non-nursing responsibilities consistent with staff training, competence, legal authorization, and policy
  • Coordinate Medicare/Medicaid coverage requirements
  • Ensure comprehensive resident care plans are established and implemented
  • Interact with physicians/providers to plan resident care
  • Ensure discharge and transfer planning
  • Interview, hire, develop, train, and retain employees
  • Plan, assign, and lead work; appraise performance; reward and coach employees; address complaints and resolve problems

Requirements

What you’ll need
  • Active Registered Nurse (RN) license in the state of practice upon hire
  • 4–6 years of experience in long-term/skilled care or similar experience
  • Ability to work the specified hours, including primarily days with night, weekend, and holiday on-call involvement
  • Visual acuity adequate to perform position duties
  • Ability to communicate effectively, hear, understand, and distinguish speech and other sounds
  • Ability to perform each essential job function satisfactorily
  • Preferred: 1–3 years of leadership experience

  • Ability to work effectively in a team environment
  • Compliance with safety principles, laws, regulations, and standards including CMS, The Joint Commission, DHHS, and OSHA if applicable
