About the role

Key responsibilities & impact
  • Utilize the nursing process to provide nursing care for Avera@Home patients in their homes or facilities
  • Collect patient health data
  • Analyze assessment data to determine nursing diagnoses or problem focus
  • Identify individualized expected outcomes and goals
  • Develop plans of care prescribing interventions to attain expected outcomes
  • Implement interventions identified in the plan of care
  • Evaluate patient progress toward outcome attainment
  • Systemically evaluate the quality and effectiveness of nursing practice
  • Collaborate with patients, significant others, and healthcare providers
  • Utilize research and evidence-based findings in practice when appropriate
  • Consider safety, effectiveness, and cost when planning and delivering patient care

Requirements

What you’ll need
  • Valid driver’s license and ability to be insured by the Avera Health automobile liability insurance carrier upon hire
  • Active Registered Nurse (RN) license in the state of practice upon hire
  • Ability to work the specified hours, including night, weekend, and holiday on-call involvement
  • Minimum two (2) years related experience and/or training or equivalent combination of education and experience (preferred)
  • Visual acuity adequate to perform position duties
  • Ability to communicate effectively, hear, understand, and distinguish speech and other sounds
  • Ability to maintain confidentiality
  • Ability to work effectively in a team environment
  • Compliance with applicable safety principles, laws, regulations, and standards, including CMS, The Joint Commission, DHHS, and OSHA
