About the role
Key responsibilities & impact- Conduct regular check-ins with the assigned patient panel via telephone
- Develop and maintain comprehensive care plans

Requirements
What you’ll need- Medical Assistant certification strongly preferred; LPN and RN also accepted
- Minimum 3 years of clinical experience in a healthcare or healthcare office setting
- Strong knowledge of chronic conditions and care management
- Proficiency in Electronic Health Records (EHR) systems and familiarity with clinical workflows
- Excellent communication and interpersonal skills
- Experience in patient education and care coordination
- Strong problem-solving skills and the ability to operate independently
