About the role
Key responsibilities & impact- Provide safety observation of assigned patients
- Maintain a safe environment and interact with patients, families, and staff in a caring and professional manner
- Provide direct patient care and assistance, including bathing, feeding, toileting, ambulating, turning, and repositioning patients
- Observe patients closely and recognize changes in condition, behavior, mood, or activity
- Report changes in the patient's condition or behavior to the nurse immediately
- Accurately document patient observations every 15 minutes throughout the shift
- Remain awake, alert, and attentive at all times during patient safety observation
- Work in all areas included within the assigned clinical grouping
- Support designated patient care and behavioral health areas across Baystate Health facilities in Western Massachusetts
- Partner with nurses and interdisciplinary teams across Baystate Health
Requirements
What you’ll need- High School Diploma or GED
- Ability to work in all areas included within the assigned clinical grouping
- CPR through the American Heart Association BLS Provider certification required prior to start
- CPI certification required within 6 months of employment
- Relevant clinical or patient care experience—at least 12 months
- Comfortable working with patients who may have behavioral health, emotional, psychiatric, or safety-related needs
- Day shift or night shift availability
- Ability to work in facility-based or system-wide assignments
