About the role
Key responsibilities & impact- Provide routine telehealth visits with Medicare patients, including E/M and Chronic Care Management

Requirements
What you’ll need- MD or DO
- Board Certified/Eligible in Family Medicine, Internal Medicine, or Geriatrics
- Active medical license
- Multi-state licensure is a plus
- Credentialed with major plans: UHC, Aetna, Humana
- Strong interest in preventive care, senior health, or chronic condition management
- Tech-savvy and comfortable practicing via telehealth
- Collaborative, mission-driven mindset with excellent communication skills
