About the role
Key responsibilities & impact- Respond to service requests during established hours
- Meet clients in their communities to assess needs and provide timely crisis support
- Conduct risk assessments and identify immediate safety concerns
- Use de-escalation strategies to stabilize clients and families experiencing a crisis
- Triage clients to determine the appropriate level of care
- Connect clients and families with treatment, stabilization services, and community resources
- Complete client-centered assessments and accurately document services in the electronic health record
- Develop individualized plans focused on risk mitigation, service connections, and ongoing support
- Provide short-term follow-up services, including psychotherapy, case management, advocacy, referrals, and resource coordination
- Follow up with clients and make referrals to stabilization services when appropriate
- Consult with internal and external providers regarding mobile response access, risk mitigation, care triage, and clinical or operational needs
- Collaborate with clients, referral sources, hospital staff, police departments, and community stakeholders
- Coordinate services and communicate across providers and systems
- Participate in team problem-solving to improve service delivery and program efficiency
- Complete clinical documentation within required timeframes
- Follow MRSS workflows, data collection requirements, agency standards, policies, and procedures
- Continue advancing clinical knowledge and training in required evidence-based interventions and fidelity model practices
Requirements
What you’ll need- BSW, MSW, or Master’s degree in Counseling with active Ohio professional license
- LISW and LPCC preferred; LSW, LPC, or MFT also considered
- Experience working in a community-based services program
- Strong clinical assessment and crisis intervention skills
- Ability to effectively de-escalate crisis situations and assess risk
- Strong communication, collaboration, and problem-solving skills
- Ability to work independently while collaborating closely with an interdisciplinary team
- Flexibility and initiative when responding to changing client and program needs
- Ability to operate your own vehicle to provide mobile-response services
