About the role
Key responsibilities & impact- Perform comprehensive nursing assessments
- Develop and implement individualized plans of care
- Provide skilled nursing services in patients' homes and residential settings
- Complete SOC, ROC, recertification, and routine visits as assigned
- Provide hospice nursing care and support to patients and families
- Monitor patient conditions and report changes appropriately
- Administer medications and treatments as ordered
- Educate patients and caregivers regarding care plans and disease management
- Coordinate care with physicians and members of the interdisciplinary team
- Communicate with the Director of Nursing regarding patient needs and changes in condition
- Maintain accurate and timely clinical documentation
- Provide compassionate support to patients and families throughout their care journey
- Coordinate directly with patients or caregivers to arrange visit times
- Accept available visits, coverage opportunities, or ongoing patient assignments based on availability and patient needs
Requirements
What you’ll need- Active RN License in the state of Texas
- Valid driver's license
- Reliable transportation
- Current auto insurance
- CPR or BLS certification
- Valid TB documentation
- Skin test within the last year, or chest X-ray within the last five years
- IV experience
- Home Health or Hospice experience is preferred
- Training requires a Monday–Friday commitment during the designated training week
