About the role

Key responsibilities & impact
  • Provide personalized hospice care to geriatric patients in their homes or residential facilities
  • Perform hospice nursing assessments
  • Complete SOCs and recertifications
  • Develop and implement individualized care plans
  • Collaborate with the Director of Nursing
  • Provide compassionate patient and family education
  • Monitor patient conditions and symptom management
  • Provide end-of-life care and create meaningful patient experiences

Requirements

What you’ll need
  • Active RN License in the state of practice
  • Valid driver’s license
  • Valid auto insurance
  • CPR or BLS Certification
  • Current TB documentation
  • Skin test within the last year OR chest X-ray within the last five years
  • Compassionate and skilled Registered Nurse
  • Ability to provide care in patient homes or residential facilities
  • Ability to serve the East and Central Henderson area
  • Typical patient visits range from 45–60 minutes
