About the role
Key responsibilities & impact- Provide skilled nursing care in patients’ homes or residential facilities
- Support RN Case Managers in implementing care plans
- Monitor patient condition and report changes
- Assist with patient education and ongoing care coordination
- Document care accurately and timely
- Maintain patient safety and quality standards
- Coordinate patient visits based on clinical needs
- Deliver care throughout a 9-week episode of care
Requirements
What you’ll need- Active LPN/LVN license in the state of practice
- Valid driver’s license
- Valid auto insurance
- CPR or BLS certification
- Current TB test (skin test within 1 year or chest X-ray within 5 years)
- New graduates and experienced nurses are welcome
