About the role
Key responsibilities & impact- Provide skilled nursing care in patients' homes or residential facilities
- Assess patient needs and report changes in condition
- Follow the established plan of care
- Work closely with the RN Case Manager and Director of Nursing
- Provide patient and caregiver education
- Monitor and document patient progress
- Communicate with physicians and members of the interdisciplinary care team
- Promote patient safety, comfort, and independence
- Complete accurate and timely clinical documentation
- Coordinate visit times directly with patients or caregivers
- Provide care to geriatric patients
Requirements
What you’ll need- Active LPN license in the state of Arizona
- Valid driver's license
- Reliable transportation
- Current auto insurance
- CPR or BLS certification
- Valid TB documentation
- Skin test within the last year, or chest X-ray within the last five years
- New graduates and experienced nurses are welcome to apply
