About the role
Key responsibilities & impact- Provide skilled nursing care to patients in their homes or residential facilities
- Assess patient needs and communicate changes in condition to the RN Case Manager and Director of Nursing
- Follow established plans of care and provide appropriate nursing interventions
- Monitor and document patient progress and care provided
- Build meaningful relationships with patients and their families
- Deliver excellent clinical care and a compassionate patient experience
- Travel to patient locations throughout the assigned service area
- Work alongside an RN Case Manager and Director of Nursing
- Support each patient throughout their 9-week episode of care
Requirements
What you’ll need- Active LPN license in the state of practice
- At least 1 year of Home Health experience
- Valid Driver's License
- Valid Auto Insurance
- CPR or BLS Certification
- AZ Level One Fingerprint Clearance Card (Arizona applicants only)
- Physical/medical clearance to perform job duties (Nevada applicants only)
- Valid TB Test — skin test within the last year or chest X-ray within the last five years
