About the role

Key responsibilities & impact

Provide personalized home health care to geriatric patients in their homes or residential facilities
Work closely with the Director of Nursing to develop individualized care plans
Support each patient's unique goals and needs
Complete 45–60 minute visits during 9-week home health episodes of care
Provide additional compassionate hospice services
Travel throughout the service area and assigned coverage territory
Participate in one weekend on-call rotation per month
Deliver high-quality, patient-centered care to rural Arizona communities

Requirements

What you’ll need

Active Arizona RN License
Valid Driver's License
Current Auto Insurance
Arizona Level One Fingerprint Clearance Card
Current CPR or BLS Certification
Current TB Test (skin test within 1 year or chest x-ray within 5 years)
Willingness to travel throughout the service area
Ability to provide coverage in the Peoria Buckeye area
Availability to take one weekend on-call rotation per month
IV experience preferred
Experience providing home health or hospice nursing care with geriatric patients
