About the role

Key responsibilities & impact

Provide one-on-one care in patients’ homes or residential settings
Provide individualized care throughout a 9-week episode of care
Perform comprehensive nursing assessments
Identify patient needs and changes in condition
Implement individualized plans of care
Provide skilled nursing interventions
Manage medications and provide education
Provide IV therapy and other skilled nursing services
Educate patients and caregivers
Monitor patient progress throughout the episode of care
Communicate changes in condition to appropriate care team members
Collaborate with physicians, Director of Nursing, and other healthcare professionals
Complete timely and accurate clinical documentation

Requirements

What you’ll need

Registered Nurse (RN)
Compassionate and dependable
Ability to provide home health care to geriatric patients throughout the Globe area
Availability for evening on-call and weekend work
Ability to perform comprehensive nursing assessments
Ability to identify patient needs and changes in condition
Ability to implement individualized plans of care
Skilled nursing intervention experience
Medication management and education
IV therapy and other skilled nursing services
Patient and caregiver education
Clinical documentation skills
Ability to communicate and collaborate with physicians, Director of Nursing, and interdisciplinary healthcare professionals
