About the role

Key responsibilities & impact
  • Partner with members and families to identify supports and direct services for health, employment, independent living, and community living goals
  • Collaborate with clinical and social support colleagues to address members’ physical, behavioral health, and long-term service needs
  • Conduct thorough, objective face-to-face home or community visits
  • Assess members’ physical, behavioral, functional, psychosocial, financial, employment, and independent-living needs
  • Use authorization criteria to approve appropriate home- and community-based services
  • Confirm authorized services are delivered and members’ needs are met
  • Manage requests and demands from internal and external stakeholders
  • Travel to members’ homes for visits

Requirements

What you’ll need
  • 3 years of experience in a clinical setting
  • 2 years of clinical experience required
  • Registered nurse with an active license in Tennessee, or an active license in the employee’s state of residence if participating in the Nurse Licensure Compact Act
  • Alternatively, Master’s in Social Work with an active unrestricted LCSW, LMSW, or LAPSW license
  • Exceptional customer service skills
  • Must live within Shelby County, Tennessee
  • Available for an 8:00am–5:00pm EST schedule
  • Valid driver’s license
  • TB Skin Test applies to coordinators working in the field
  • Basic Microsoft Office and email skills
  • Effective time management skills
  • Excellent oral and written communication skills
  • Strong interpersonal and organizational skills
  • Must meet BCBST Driver Safety Program requirements when routinely operating a company, personal, or rental vehicle, including valid driver’s license, compliant auto insurance, and acceptable motor vehicle record
  • Must complete a digital literacy assessment
  • Various immunizations and/or associated medical tests may be required
