About the role
Key responsibilities & impact- Partner with members and families to identify supports and direct services for health, employment, and independent or community living goals
- Collaborate with clinical and social support colleagues to meet members’ physical, behavioral health, and long-term service needs
- Conduct thorough, objective face-to-face visits and assess members’ physical, behavioral, functional, psychosocial, financial, employment, and independent-living needs
- Use criteria to authorize appropriate home- and community-based services
- Confirm authorized services are provided and members’ needs are being met
- Travel to members’ homes for visits
- Manage requests and demands from internal and external stakeholders
Requirements
What you’ll need- 3 years of experience in a clinical setting
- Registered nurse with an active license in Tennessee, or an active license in the candidate’s state of residence if participating in the Nurse Licensure Compact Act; or Master’s in Social Work with an active unrestricted LCSW, LMSW, or LAPSW license
- Must live in Dickson County or Hickman County, Tennessee
- Available for an 8:00am–5:00pm EST schedule
- Valid driver’s license
- TB Skin Test for coordinators working in the field
- Various immunizations and/or associated medical tests may be required
- Digital literacy assessment required
- At least 2 years of clinical experience required
- Basic PC skills, Microsoft Office, and email
- Effective time management skills
- Excellent oral and written communication skills
- Strong interpersonal and organizational skills
