About the role

Key responsibilities & impact
  • Support members with complex healthcare needs through safe discharge planning and coordinated post-acute care
  • Conduct face-to-face member visits in area hospitals
  • Collaborate with hospital staff, physicians, specialists, facility staff, and internal care teams
  • Identify member needs, remove barriers to care, and connect members with appropriate resources
  • Reduce gaps in care and support members through transitions from acute care to home and community-based care
  • Support utilization management functions for complex and non-routine cases
  • Serve as a liaison between members, providers, and internal/external customers
  • Oversee complex cases and implement interventions to ensure efficient benefits utilization
  • Perform case management activities including assessment, planning, implementation, coordination, monitoring, outcomes, and evaluation

Requirements

What you’ll need
  • Registered Nurse (RN) with an active license in Tennessee, or a license in the state of residence if participating in the Nurse Licensure Compact Law
  • 3 years of clinical experience required
  • 5 years of experience in the health care industry
  • For Select Community & Katie Beckett: 2 years of experience in IDD for Select Community required
  • Excellent oral and written communication skills
  • Basic Microsoft Office and email skills
  • Ability to live within 50 miles of Knoxville, TN
  • Ability to travel to Knoxville-area hospitals at least one day per week
  • Sponsorship is not available for this role
  • Ability to communicate effectively via digital channels and provide technical support
  • Digital literacy assessment required
  • Valid driver's license, compliant auto insurance, and acceptable motor vehicle record if routinely operating a company, personal, or rental vehicle for company business
