About the role

Key responsibilities & impact
  • Support members’ physical, behavioral, and social health and well-being
  • Assist members in finding and maintaining competitive, integrated employment
  • Navigate challenging relationships to ensure members receive necessary services
  • Perform intake, enrollment, and ongoing assessments
  • Develop and maintain Person-Centered Plans
  • Monitor and maintain overall service provision
  • Partner with members and families to identify needed supports and direct services aligned with personal goals
  • Collaborate with clinical and social support colleagues to meet members’ physical, behavioral health, and long-term service needs
  • Conduct thorough, objective face-to-face visits and assess members’ physical, behavioral, functional, psychosocial, financial, employment, and independent-living needs
  • Apply criteria to authorize appropriate home- and community-based services and confirm services are delivered and members’ needs are met
  • Travel to members’ homes across the Cumberland, Roane, Morgan, and Blount County area

Requirements

What you’ll need
  • RN with preference for current Developmental Disabilities Nurses Association certification as a Certified Developmental Disabilities Nurse (CDDN) for RNs or Developmental Disabilities Nurse (DDC) for LPNs
  • Or B.S. in social work, nursing, education, human service, health care profession, or another related field
  • Licensed Master Social Worker or Licensed Clinical Social Worker preferred
  • Or meet federal requirements for a Qualified Developmental Disabilities Professional (QDDP or QIDP)
  • Or, in lieu of a degree, 5+ years’ experience as an independent Support Coordinator or case manager for service recipients in a 1915(c) HCBS Waiver, plus completed Personal Outcome Measures Introduction and Assessment Workshop trainings
  • 2 years of experience in the healthcare industry or an operations-type environment required
  • Valid driver's license
  • Various immunizations and/or associated medical tests may be required
  • Digital literacy assessment required
  • Proficiency in Microsoft Office, including Outlook, Word, Excel, and PowerPoint
  • Ability to work independently with minimal supervision or in a team environment
  • Problem-solving ability and capacity to manage multiple assignments with critical deadlines
  • Strong oral and written communication skills
  • Strong interpersonal and organizational skills
  • Ability to be organized, reliable, and manage multiple tasks
  • For routine vehicle use: valid driver’s license, compliant auto insurance, and acceptable motor vehicle record
  • Availability for an 8:00am–5:00pm schedule
