About the role

Key responsibilities & impact

  • Assist members in finding and maintaining competitive, integrated employment
  • Navigate challenging relationships to ensure members receive necessary services
  • Perform intake, enrollment, and ongoing assessments
  • Develop and maintain Person-Centered Plans
  • Monitor and maintain overall service provision
  • Partner with members and families to identify supports and direct services supporting health, employment, and independent or community living
  • Collaborate with clinical and social support colleagues to address members’ physical, behavioral health, and long-term service needs
  • Conduct thorough, objective face-to-face visits and assess members’ physical, behavioral, functional, psychosocial, financial, employment, and independent-living needs
  • Apply criteria to authorize appropriate home- and community-based services and confirm services are provided and members’ needs are met
  • Travel throughout Sumner County to members’ homes

Requirements

What you’ll need
  • RN; or B.S. in social work, nursing, education, human service, health care profession, or related field; or meet federal QDDP/QIDP requirements
  • In lieu of a degree: 5+ years’ experience as an independent Support Coordinator or case manager for service recipients in a 1915(c) HCBS Waiver, plus completed Personal Outcome Measures Introduction and Assessment Workshop trainings
  • 2 years of experience in the healthcare industry or an operations-type environment required
  • Current CDDN certification preferred for RNs, or DDC certification for LPNs, as applicable
  • Licensed Master Social Worker or Licensed Clinical Social Worker preferred
  • Valid driver's license
  • Various immunizations and/or associated medical tests may be required
  • Digital literacy assessment required
  • Proficient in Microsoft Office (Outlook, Word, Excel, and PowerPoint)
  • Ability to work independently or in a team environment
  • Problem-solving ability and capacity to manage multiple assignments with critical deadlines
  • Organization, reliability, strong work ethic, and ability to manage multiple tasks
  • Excellent oral and written communication skills
  • Strong interpersonal and organizational skills
  • Drivers operating a vehicle for company business must comply with the BCBST Driver Safety Program, including valid license, auto insurance compliance, and acceptable motor vehicle record
  • Ideal candidates live in Sumner County or within 35 miles
