About the role

Key responsibilities & impact
  • Coordinate patient transfer logistics, patient acceptance, consultation, access, and bed placement
  • Identify the appropriate level of care, accepting provider, and destination plan for referred patients
  • Work with clinicians and providers to communicate accurate clinical presentations and patient information
  • Ensure timely access, appropriate bed assignments, accurate documentation, quality, safety, and efficiency
  • Use EHR, telephony, and office systems while protecting PHI
  • Apply problem-solving, critical thinking, and clinical decision-making to recommend patient outcomes
  • Act as liaison among patients, physicians, hospital staff, referring facilities, receiving facilities, and outside agencies
  • Monitor individual and team compliance and performance
  • Support process changes and regulatory and quality initiatives
  • Identify, trend, and report barriers to patient access and throughput
  • Report safety, compliance, and operational risks through the defined chain of command
  • Participate in process improvement, professional development, peer development, peer review, and shared leadership
  • Assist with quality data collection
  • Maintain active RN licensure within the states where transfer center services are provided
  • Work assigned shifts, including every other weekend, on-call coverage, and rotating holidays
  • Perform other tasks and analysis as requested or directed

Requirements

What you’ll need
  • Active Registered Nurse (RN) license required
  • Active multistate/compact registered nurse (RN) licensure required if applicable based on state of residence
  • Associate Degree of Nursing or diploma (ADN) required
  • 1 year of acute care experience required
  • Must have a designated workspace with a locked door, per HIPAA regulations
  • Secondary work location encouraged, meeting HIPAA regulations
  • Minimum internet speed of 100 Mbps download and 20 Mbps upload at primary and secondary work locations
  • Proficiency in Microsoft Office tools, including Outlook, MS Teams, Excel, and Word
  • Proficiency in EHR data entry, telephony systems, and office systems
  • Knowledge of EMTALA, Ethics and Compliance, quality initiatives, and HIPAA
  • Ability to review and interpret patient transfer case information
  • Ability to escalate and inform regarding patient status changes
  • Ability to record tasks and conversations regarding patient transfers
  • Critical thinking and clinical decision-making skills
  • Excellent oral and written communication skills
  • Active listening, teamwork, relationship building, multitasking, attention to detail, and adaptability
  • Epic electronic health record experience preferred
  • Genesys contact center experience preferred
  • Healthcare contact center experience preferred
  • Bachelor's degree of nursing (BSN) preferred
  • 3+ years of acute care experience preferred
