About the role
Key responsibilities & impact- Provide telephonic triage to determine the most appropriate level of care
- Provide requested support and/or virtual monitoring
- Offer recommendations, education, and care advice using decision-making tools, clinical judgment, workflows, protocols, and resources
- Participate in care coordination to reduce readmissions, enhance chronic disease management, manage health risk and injury reporting
- Schedule provider appointments and facilitate provider communication
- Ensure accurate and timely documentation in the EMR according to best practices, guidelines, and workflows
- Participate in virtual monitoring, reporting, and escalation to identified support services
- Support Conduit Health Partners business functions and continuity of operations
- Participate in process improvement, professional development, peer development, and peer review
- Work evenings, weekend rotations, and three holidays per year
- Maintain performance standards prioritizing safety, quality, patient experience, and organizational strategic initiatives
Requirements
What you’ll need- RN license required in applicable state(s)
- Multistate/Compact RN Licensure preferred
- ADN or Diploma in Nursing required
- BSN preferred
- 1 year of acute care nursing experience required
- Triage experience preferred
- EPIC Electronic Health Record preferred
- Ability to provide care appropriate to neonates, infants, pediatrics, adolescents, adults, and geriatrics
- Minimum internet speed of 100 Mbps download and 20 Mbps upload at primary and secondary work locations if working remotely
- Ability to perform shift work
- Required to travel by car to off-site locations occasionally, including in adverse weather conditions
- Manual dexterity, finger dexterity, color vision, far and near acuity, and ability to hear alarms, telephone, and audio recordings
