About the role
Key responsibilities & impact- Provide social work services to patients on an intermittent basis in their places of residence
- Work in accordance with physician orders and plans of care under the supervision of the Director of Clinical Services/Nursing Supervisor
- Assess patient/family system needs
- Develop and implement treatment plans according to departmental and medical center policies
- Perform bio-psychological assessment interviews with patients, families, or significant others
- Provide crisis intervention/treatment in emergency situations, including assessment, counseling, information/referral, and consultation to physicians and the healthcare team
- Participate in developing Plans of Care and prepare clinical and progress notes
- Submit assessments and notes timely
- Interpret patients’ conditions professionally and recommend appropriate psychosocial interventions or treatment plans
Requirements
What you’ll need- A Master’s or Doctoral Degree from a school of social work accredited by the Council on Social Work Education
- Two (2) years of Social Work experience in a Home Health setting
- Passion to serve and help others live their best lives possible
- Ability to provide social work services in patients’ places of residence in accordance with physician orders and plans of care
- Must be able to assess patient/family system needs
- Must be able to develop and implement treatment plans
- Must be able to perform bio-psychological assessment interviews
- Must be able to provide crisis intervention/treatment to adults, children, and families
- Must be able to prepare clinical and progress notes and submit assessments and notes timely
- Must be able to provide professional interpretations of patients’ conditions and recommend psychosocial interventions or treatment plans
