About the role

Key responsibilities & impact
  • Administer skilled nursing care to patients on an intermittent basis in their place of residence
  • Make the initial evaluation visit and regularly reevaluate the patient’s nursing needs
  • Meet Daily Productivity Standards
  • Initiate the plan of care and necessary revisions
  • Provide services requiring substantial specialized nursing skills
  • Initiate appropriate preventative and rehabilitative nursing procedures
  • Prepare clinical and progress notes for each patient visit
  • Prepare timely summaries of care conferences on patients
  • Personalize and coordinate care to support patient recovery, wellness, independence, and quality of life
  • Work under physician orders and the plan of care under the direction of the Director of Patient Care Services

Requirements

What you’ll need
  • Graduate of an accredited school of professional nursing
  • Currently licensed as a registered nurse in the state of the job posting
  • Minimum of one (1) year of nursing experience in a home health setting
  • Must have current BLS Certification
  • Access to reliable transportation
  • Preferably resides in Strongsville, Brook Park, Parma, Seven Hills, or Independence area
  • Passion to serve and help others live their best lives possible
