About the role

Key responsibilities & impact

Serve as the primary point of contact for a caseload of home health patients
Ensure seamless coordination of services
Complete OASIS assessments
Develop individualized care plans with the interdisciplinary team
Coordinate with physicians, nurses, therapists, social workers, and home health aides
Track patient progress and update care plans
Communicate with families and caregivers as needed
Maintain accurate and timely clinical documentation according to Medicare guidelines and agency policy
Provide education, resources, and emotional support to patients and families
Promote patient independence and wellness

Requirements

What you’ll need

RN (Registered Nurse) with active (state) license
At least 1 year of experience in home health or case management preferred
Strong critical thinking, communication, and time management skills
Ability to work independently
Comfortable using EHR systems
Experience with Kantime, Axxess, HCHB, or WellSky is a plus
Valid driver’s license
Auto insurance
Access to a dependable vehicle
Comfortable completing OASIS assessments and developing individualized care plans
