About the role
Key responsibilities & impact- Provide care and services for a group of patients
- Visit patients where they reside, including private homes, Skilled Nursing Facilities, and assisted living facilities
- Contribute as a member of the interdisciplinary team
- Provide expertise regarding psych-social assessments and interventions
- Apply knowledge of community sources
- Work in a Case Management, Admissions, or Resource role
- Communicate by phone, email, and in writing
- Attend meetings with healthcare professionals
- Manage visits and documentation
- Respond to urgent needs by prioritizing and triaging multiple concerns
Requirements
What you’ll need- Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice
- Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership
- Work closely with patients on a 1-1 level while receiving support from a patient care team
- Ability to work independently on a day-to-day basis
- Phone, email, and written communication skills
- Ability to attend meetings with a variety of healthcare professionals
- Understanding of hospice rules and regulations
- Time management skills to manage visits, documentation, and meetings
- Ability to respond to urgent needs by prioritizing and triaging multiple concerns
- Ability to learn an electronic medical records system
- Willingness to drive with reliable transportation, valid driver’s license, and auto insurance to travel across the service area
