About the role

Key responsibilities & impact
  • Provide oversight, supervision, and support to field staff including CNAs, CHHAs, and other caregivers
  • Conduct patient assessments and develop individualized care plans according to professional nursing standards
  • Collaborate with physicians and multidisciplinary team members to optimize patient care and outcomes
  • Educate patients and families on health maintenance, disease prevention, and care expectations
  • Ensure accurate documentation, regulatory compliance, and adherence to Joint Commission standards
  • Monitor clinical performance and provide coaching, training, and mentorship to staff
  • Liaise between patients, families, staff, and the BrightStar Care leadership team
  • Support case management, scheduling, and follow-up to ensure continuity of care

Requirements

What you’ll need
  • Active RN license in the state of Massachusetts
  • Minimum 2 years of nursing experience
  • Home health or acute care experience strongly preferred
  • Prior leadership, supervisory, or case management experience is a plus
  • Strong organizational, interpersonal, and communication skills
  • Passion for delivering high-quality, patient-centered care
  • Active CPR certification
  • Valid driver’s license
  • Legally authorized to work in the US
  • Willing and able to commute to the office
