About the role
Key responsibilities & impact- Provide direct services to individuals and families needing crisis intervention and eviction prevention services
- Conduct intake, engagement, assessment, service plan development, referrals, advocacy, exit summaries, and follow-up
- Manage a caseload for crisis intervention and eviction prevention services
- Connect clients to government benefits and entitlements that support stable housing
- Monitor and document client progress toward service plan goals in Salesforce
- Document all client interactions in Salesforce
- Maintain familiarity with on-site and off-site program resources
- Coordinate services with social services, mental health, medical, employment, educational, childcare, and other providers
- Conduct case conferences with clients and service providers
- Complete monthly or otherwise required program reports
- Report to and meet with the supervisor monthly
- Conduct home visits, outreach events, and other fieldwork
- Develop customized service plans with clients
- Assist clients with documentation for benefits, renewals, and recertification
- Maintain current case applications, Salesforce records, and regular client contact
- Coordinate referrals for needed services
- Provide budget counseling and support effective money management
Requirements
What you’ll need- Bachelor’s Degree preferred
- High school diploma/GED with 1 year experience in benefits and government entitlements
- Bilingual verbal and written skills preferred
- Proficiency with personal computers, electronic mail, voicemail, and copier equipment
- Proficiency in virtual platforms preferred
- Ability to use a computer for prolonged periods
- Ability to occasionally lift and/or move up to 10 pounds
- Ability to stand, walk, or sit for long periods of time
- Ability to bend and retrieve objects and/or documents
- Ability to commute within the Bronx
- TB Test required within the first 120 days of employment for Shelters and Homeless Outreach programs
