About the role

Key responsibilities & impact
  • Lead investigations of patient safety events and other significant organizational incidents
  • Review assigned incidents and establish investigation plans
  • Determine scope and investigative approach based on event type and risk level
  • Prioritize investigations according to organizational and regulatory requirements
  • Interview team members and other involved individuals
  • Collect and analyze clinical documentation and supporting evidence
  • Facilitate Root Cause Analyses (RCA), Apparent Cause Analyses (ACA), and mortality reviews
  • Identify system issues and contributing factors
  • Develop recommendations and corrective action plans
  • Partner with operational leaders to implement corrective actions
  • Monitor corrective action implementation
  • Participate in enterprise safety committees
  • Collaborate with the Patient Safety Organization (PSO) by providing incident information for protected patient safety analysis
  • Provide consultation regarding incident reporting and investigation processes
  • Support organizational responses to regulatory inquiries, surveys, and incident-related information requests
  • Maintain investigation documentation
  • Identify opportunities for organizational improvement
  • Perform other duties as assigned

Requirements

What you’ll need
  • Bachelor's degree in Nursing, Behavioral Health, Healthcare Administration, Social Work, Psychology, or related healthcare field, or equivalent combination of education and relevant experience
  • Three years healthcare experience
  • Knowledge of patient safety principles
  • Knowledge of Root Cause Analysis methodologies
  • Systems thinking
  • Strong interviewing skills
  • Clinical judgment
  • Excellent communication
  • Ability to manage sensitive situations professionally
  • Strong analytical and critical thinking skills
  • Ability to influence without direct authority
  • Experience in quality, patient safety, clinical operations, compliance, or risk management preferred
  • Experience participating in incident investigations preferred
  • CPHQ certification preferred
  • CPPS certification preferred
  • Licensed clinical professional preferred
